Bayshore Health Care & Rehabilitation Center

Bayshore Health Care & Rehabilitation Center is a Medicare and Medicaid-approved skilled nursing facility with 190 long-term beds housing a separate 11-bed Ventilator Unit and a dedicated sub-acute care unit. Experienced geriatric professionals who offer residents compassionate care in a pleasant, home-like setting, staff each unit.  

A 52-bed Comprehensive Personal Care Home is also contained within our building. The Center's central location on the hospital's campus offers residents immediate access to acute care and diagnostic services.  

Bayshore Health Care & Rehabilitation Center also houses a Sub-acute Care Unit that is supported by our state-of-the-art rehabilitation therapy department and staffed by a team consisting of physical therapists, occupational therapists, speech and language pathologist, and a physiatrist. The team sets goals that support residents' individual discharge plans, enabling them to return to their homes following an acute illness that may require rehabilitation services to restore functional independence.  

Overview of Services

  • Semi-private rooms with bathrooms; private accommodations are available
  • Three restaurant-style meals are served daily in our dining rooms or in-room service is provided if needed
  • We are staffed 24-hours per day by RN's, LPN's and Certified Nursing Assistants who monitor medications and develop health care plans to coordinate care and assist with activities of daily living such as bathing, dressing, walking and eating, as indicated
  • Transportation services can be arranged for off-site physician and diagnostic services
  • A separate 52-bed Comprehensive Personal Care Home provides assisted living services reimbursed through the Medicaid waiver program or private payment

Comprehensive Personal Care Home

A separate 52-bed Comprehensive Personal Care Home housed within Bayshore Health Care Center, provides assisted living services reimbursed through the Medicaid waiver program or private payment.  

Assistance with activities of daily living, medication management and wellness services are provided. Residents are provided a safe living environment where caregivers are available to help with daily hygiene, grooming and simple housekeeping for those who require such assistance.  

Residents visit their private physician and participate in housekeeping needs of their rooms, while being provided the level of assistance and supervision they need, meals and social opportunities in a protective environment.  

Other services available within the Meridian Health continuum of care are easily accessible for all residents. To most who reside there, it is their true home where they are safe and part of the family they have become.  


Sub-Acute Care at Bayshore Health Care Center

Bayshore Health Care & Rehabilitation Center provides sub-acute care services for patients being discharged from the hospital following an acute illness and who require intensive rehabilitative services to enable them to return to their homes.  

Individualized rehabilitative plans are developed for all residents who are admitted to our facility to assist them in achieving their desired discharge goals. Our staff of rehabilitation professionals work closely to ensure that each resident's functional capacities are restored to the most optimum level to enable them to return home, and function safely and independently.  

Our therapists work closely with our physiatrist and the attending physician, in conjunction with other members of the health care team, to develop reasonable goals to facilitate discharge after a short-term stay. Often rehabilitation services are continued in the home or in the community through visiting nurse or outpatient sources to continue the progress and maintain function.  

We accept payment from varied sources such as Medicare, private insurance and HMOs. We require an application to be submitted prior to admission and, if required, a letter of authorization from the insurance provider. This is usually arranged by the Hospital case manager in conjunction with our admission and social services staff once request for admission is received.  


Ventilator Care Unit

The Ventilator Unit at Bayshore Health Care & Rehabilitation Center is licensed as a skilled nursing facility by the State of New Jersey Department of Health. It is currently the only long-term ventilator unit of its kind in Monmouth County.

Our staff has been carefully selected on the basis of experience, competence and, most of all, their desire to exhibit a supportive attitude and provide quality care to those individuals who require mechanically assisted ventilation on a daily basis.

It is the responsibility of our staff to create a safe, supportive environment that will help our residents maximize their potential and live their lives to the fullest capacity.  

Twenty-four hour nursing care is provided by registered nurses, LPN's, certified nursing assistants and respiratory therapists, supplemented by a team of physical and occupational therapists, and a speech pathologist. They are guided by our pulmonologist and physiatrist, who coordinate medical services with each resident's attending physician to optimize care delivery. Working closely with residents, staff and families, they help provide education in the special needs of the resident on a ventilator.

We consider our unit to be unique, and are proud of the dedication our staff shows to all residents and their family members who are faced with the limitations and challenges of ventilator dependency.
