Diabetes – The Challenge
People with diabetes have many options for controlling elevated blood sugars. Today's modern medicines, plus proper nutrition, exercise, and self blood glucose monitoring allow for better blood sugar control. The result – decreased long-term complications and longer lives for those people diagnosed with diabetes. The challenge – the sheer number of options and day-to-day responsibilities can often seem overwhelming. The Meridian Pharmacology Institute can offer help, guidance, and education.

The Meridian Pharmacology Institute has established a service for people with diabetes to help navigate the confusing issues of diabetes care. Combining the knowledge and expertise of the consultant pharmacist with available new technology, we can help guide you to better outcomes through enrollment in our Diabetes Monitoring Program.

If you use a blood glucose meter to test your blood sugar, you can access our new technology that can optimize your diabetes care and enhance your quality of life.

Diabetes Monitoring and Education
We can create helpful reports and graphs from your blood glucose meter data that can be printed and shared with your physician. These reports and graphs can identify trends or pinpoint where some changes might be beneficial.

Our Diabetes Monitoring Program includes:

  • Private Consultation with Consultant Pharmacist
  • Medication Therapy Management (RxCheck)
  • Personal Medication Record (PMR)
  • Medication Action Plan (MAP)
  • Supplemental diabetes education
  • Communication and coordination with your diabetes educator, physician, and dietician

The MetrikLink®
iMetrikus MetrikLink® Technology allows you to upload stored data from the memory of your existing blood glucose meter over your phone line for regular review and recommendations by the consulting pharmacist. Uploaded data can also be reviewed by you, your physician, your family, a caregiver, and our clinical pharmacist.
