Admission & Discharge

Admission Criteria
Once an Inpatient referral is made, our admission liaison assesses the rehabilitation needs of the patients in consultation with the patients, families, referral sources and our Medical Director. After the assessment is completed, we notify the referral source if the services available at SRI will best meet the needs of the patient or suggest another program that may be better suited.

Each individual is different but to be considered a good candidate for admission to SRI an individual must demonstrate medical stability, need, willingness and ability to tolerate and benefit from at least three hours of therapy five days a week in an interdisciplinary environment which includes rehabilitation physician, rehabilitation nursing and case management.

The therapies are a combination of therapies such as Physical, Occupational, Speech and Prosthetics and Orthotics.

We modify patients rehab plan to accommodate the cultural preferences as well as cognitive and behavioral needs of the patient to optimize recovery. Please ask our admission liaison or a team member at SRI if you have questions.

After Discharge
Our goal is to improve function and discharge our patients into the community with continued Outpatient therapy. Some patients may need continued rehabilitation stay in a less intense environment or other services on an ongoing basis or a support group after discharge. Our team will work with you to determine the most appropriate setting for you to continue your recovery.
