Every day, the Meridian Health family works enthusiastically to help those in the community seeking our services. Whether your family has a sick child, an expectant mother, or an ailing grandparent, we will be here for you and your loved ones because like you, we understand family is everything.

Comprehensive Care
Family Resource Centers at Meridian HealthMeridian Health's Family Resource Centers provide a wide range of acute and preventative health services to meet the needs of every family member.

Eligibility and Appointment Information
If you have Medicaid, Medicare, or any type of insurance coverage, please call 732-776-4209 for instructions on setting up an appointment. Appointments are required, and if you are a member of a Health Maintenance Organization (HMO), your primary care physician must be a doctor with the Family Health Center for you to be eligible to use our services.

There is a charge for all medical services. If you do not have insurance, unless you are pregnant, you must first complete a financial evaluation prior to seeing one of our doctors. Interviews are conducted Monday through Thursday in the Financial Assistance (Charity Care) Office. Please bring the following documents with you to help expedite the process:

  • Proof of income (three months of pay stubs)
  • Proof of address (driver's license or envelope showing your current address)
  • Social Security card
  • Any type of insurance card
  • Checking and savings account statements

A comprehensive financial evaluation and determination process will be provided during your interview. While in the process of applying for Charity Care, patients can also schedule an appointment.

Medicare and Charity Care Patients
The New Jersey Department of Human Services has instituted an insurance program called "Family Care." You may become part of an HMO, and you will need to choose a primary care physician for yourself and your family. For more information on this program, please call 1-800-701-0710.
